During the first quarter of 2026, Endura Healthcare, LLC was the owner of Boulder Canyon Health and Rehabilitation, Centers for Medicare and Medicaid Services (CMS) records show. Endura Healthcare, LLC has held ownership of the Boulder County facility since Dec. 15, 2020.
The facility is classified as a for-profit nursing home.
Throughout that period, Boulder Canyon Health and Rehabilitation maintained an overall CMS rating of 4, exceeding the state nursing home average of 3.2 in Colorado. CMS assigns ratings based on health inspections, staffing, and the quality of resident care.
Statistics for the first quarter of 2026 indicate Boulder Canyon Health and Rehabilitation cared for an average of 123.1 residents daily, with 140 beds on site. It is not a hospital-based facility.
CMS data also report that the facility recorded no fines or penalties in the first quarter of 2026.
Boulder County had eight for-profit nursing homes in total.
Figures from the U.S. Census Bureau show the nation’s older population rising while younger age groups are shrinking. From 2020 to 2024, Americans age 65 and older increased by 13%, and the population under 18 dropped by 1.7%. Over the past century, the older adult population in the United States grew dramatically, from 4.9 million in 1920 to more than 61 million by 2024.
A 2021 study estimates that almost half of adults in this age range—about 28 million—could need long-term services and support, with projections indicating more than one-third will ultimately require care in a nursing home.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
